CHU Implemented HIV/TB Project on : Community Health Systems Strengthening for Increased Access to quality HIV/AIDS and TB services in Kole, Oyam and Kwania districts with funding from USAID JCRC/LPHS-Kigezi &Lango from October 2021-July2023.
Key Activities included;
- Support to conduct high yield HTS modalities at facility and community focusing on index client testing and APN.
- Conducting mentorship and supervisions for health workers and community structures.
- Conducting Integrated outreaches in hotspot areas.
- Facilitating CLFs to follow up of lost clients and missed appointments
- Establishing of 12 new CDDPs and support to MCCLADs created.
- Conducting home-based IAC and psychosocial support to vulnerable clients who cannot come to the health facilities.
- Conducting Paed, Adolescent and caregivers’ meetings at community level
- Conducting community Viral load bleeding through integrated outreaches and home visits
- Condom distribution.
- Conducting TB contact tracing for all contacts of biologically confirmed positive clients.
- Conducting TB hotspot screening integrated with Community awareness creation on HIV/AIDS causes, TB signs and symptoms.
- TPT initiation
- Conducting Integrated quarterly review meetings across the supported facilities in the 3 districts.
KEY ACHIEVEMENTS
- Improved 3 months’ cohort retention to 98% above UNAID target of 95%.
- Initiated 1695 PLHIV on TPT (71%).
- Tested 37584 clients for HIV,
- Identified 1390 HIV+ clients and linked to care.
- Followed Up and returned to care 3378 PLHIV
- A total of 2,676 community members screened for TB and 2,6,24 initatied on TPT.
- Referred 11829 pregnant mothers and 629 exposed infants for eMTCT services.
- Distributed 233 HIV self-testing kits to individuals
